RE:active Card Procedure & Eligibility Criteria:

The RE:active Card offers leisure discounts to Looked After Children

  1. Initially all Kingston’s Looked After Children (living within Kingston or neighbouring boroughs) eligible from the LAC register will be sent an information flyer with attached tear off application form. LAC will also be able to access Application Forms from Kingfisher Leisure Centre or the Children's Participation & Development Officer. Local Kingston Foster Carers will also be sent a letter with an attached application form to assist those they Care for to complete it.

  2. LAC complete the tear off application form and send to: Strategic Manager for Looked After Children's Services, Room 203, Guildhall 1, Kingston KT1 1EU. They must also enclose with their application form 2 passport size photographs (1 will be used on the card & the other will be kept on file with completed application form by Kingfisher.)

  3. If the application form & photo's have been given to the LAC’s Social Worker, the Social Worker must check the details are correct & forward on to the Strategic Manager for Looked After Children's Services for an Authorisation Signature.

  4. Once the application form & photo's have been checked (i.e Check identity, date of birth is correct/address etc.) The Strategic Manager for Looked After Children's Services signs the application form giving authorisation. The authorised application form & photo's are then sent to Kingfisher Leisure Centre, where the application forms and photo’s are processed into RE:active Cards.

  5. Kingfisher keep a Confidential record of the application form from each LAC, produce the card with ID photo on it, Laminate card & send it to the LAC using the address given on the application form.

  6. Katie Goodyear, The Children & Youth Participation Worker, will visit Kingfisher on a quarterly basis to ensure that the LAC details on file are still correct (i.e not moved house/ not turned 18/ still in Care etc.)

  7. Once a LAC turns 18, leaves Care or is no longer Looked After by RBK then their card will be invalid and they will not be able to access these discounts. In time we will look at how well this model works, will look at providing more discounts and will consider a similar initiative for Care Leavers.

  8. When a RE:active Card is presented at any one of the providers involved in the Scheme, the child’s photo identity & age must be checked and the usage record on the appropriate ‘button on the till.’ All co-operating sites have been asked to set up the 'buttons on the till' for this scheme, so that the various activities are charged at the RE:active Card Reductions rate and that usage can be recorded for reporting to the Partnership Board.
